Elementary Application Form

*Required*  This form submits your request for your student's enrollment. The registrar will then confirm your student's  status - $80 Application Fee per Student

Elementary Medical Form

*Required*  Use this form to share any medical needs, allergies, or health considerations so staff can properly support your child during the day.

*Required* This form identifies emergency contacts and authorized pick-up individuals, ensuring your child is released safely and according to KDO procedures

HS/Bridge Medical Form

*Required*  Complete this form to communicate medical needs, allergies, or health concerns that may impact your student during the school day.

2026 Graduation Form

*Opens in the fall of each year* For seniors, this form gathers information for graduation planning, including ceremony participation, cap and gown ordering, and related logistics.

High School / Bridge

Please Note:

Application opens and closes at the times indicated. Application forms submitted early will not be accepted and will have to be resubmitted at the correct time.  All forms must be submitted at the time of application. You will receive a confirmation email from the Registrar confirming your requested class(es) within 1 week of application submission. Previous attendance at KDO does not guarantee a spot for your student(s); classes have space limitations and some fill up quickly.

This guide helps families choose a balanced set of classes by grade level while leaving room for flexibility in their homeschool plan.

Payments, Invoices, Fees

Ways to Submit Payment:

  • Mail to: KDO Academy, 2536 Eastern Blvd, Unit 131, York PA 17402

  • Drop off in the clear box at the front desk.

  • Use bill pay from your bank

  • PayPal: Send payment to info@kdoacademy.org. NOTE: PayPal payments incur a 4% fee.  Manually add the fee to the amount you wish to pay. 

Please Note:
     Please include your student’s name(s) on every payment!Reminder: $25 fee for bounced checks
